Private Services is the fee-for-service component of the CFPD and offers:
Who We Serve

We work with people who have a wide range of disabilities, from developmental to physical disabilities or mental illness. Coordinating the many services involved in the person's daily life, such as residential, medical, financial and vocational, we are focused on serving the needs of each individual. We excel in working with:
  • Clients and Families
  • Trustees and Bank Trust Departments
  • Attorneys and Other Professionals

PRIVATE CASE MANAGEMENT

We are committed to establishing long term relationships with individuals to secure quality care, services and support throughout their lifetime. We draw on years of experience and strong community connections to assist clients in developing a comprehensive plan for their care. Well versed in Supplemental Needs Trusts (SNT's), we will work collaboratively with the beneficiary; their family members; trustee and trust departments; service providers; attorneys and others to meet their needs while maintaining their eligibility for public benefits and improving their quality of life. In particular, we offer the following Case Management Services:
  • Long-Term/Ongoing
  • Short-Term/Assessment and Planning
  • Respite/Referral
  • Funds Management

CONSERVATORSHIPS

CFPD excels as a conservator in cases involving people with disabilities, the elderly or other individuals who need a long-term relationship. Our expertise in fund investment and management, coupled with our Assessment and Planning for individuals gives us a unique advantage in serving as conservator. 

TRUST ADVISORY SERVICES

CFPD works with trustees and bank trust departments to make recommendations related to the SNT. In this case, the trustee/bank can focus on investments and we use our expertise to help maintain the beneficiary's public benefits such as SSI and Medicaid. We can also assist a family member who wants to act as trustee by advising them on an as needed basis.

MEDICARE SET ASIDES

With years of experience in Medicare and Medicaid services, we offer you reliable counsel in the complex world of Medicare Set Asides. Our reliable staff has completed the MSA Training Course, giving them the knowledge to help beneficiaries maintain their Medicare eligibility. In particular, we offer the following services:
  • Medicare Set-Aside Allocation Reports
  • Management of Medicare Set Aside Accounts/Trusts
  • Medicare Set Aside Trusts
